Our pledge regarding your health information

Rome Radiology Group is committed to protecting the privacy of your protected health information ("PHI"). PHI is information about you, including basic demographic information, that may identify you and that relates to your past, present, or future physical or mental health condition and related health care services. We are required by law to maintain the privacy of PHI, provide you with this notice of our legal duties and privacy practices, and follow the terms currently in effect.

How we may use and disclose your health information

The following describes the ways we may use and disclose your PHI without your written authorization. Not every use or disclosure will be listed, but every permitted use will fall within one of these categories.

For treatment

We use and disclose your PHI to provide, coordinate, or manage your radiology care and any related services. This includes sharing your imaging studies and reports with the physician who ordered your exam, with other physicians and providers involved in your care, and with the imaging center where your exam was performed.

For payment

We use and disclose your PHI so that we, the facility where your study was performed, and your health plan can bill and receive payment for the services provided to you. This may include sharing information with your insurance company, our billing partner, or other entities involved in payment.

For health care operations

We use and disclose your PHI to support the business activities of our practice, including quality review, peer review, accreditation, staff training, credentialing, and similar activities.

Other permitted and required uses and disclosures

We may also use or disclose your PHI without your authorization in certain other circumstances, including:

  • When required by law, court order, or subpoena
  • For public health activities, such as reporting communicable diseases
  • To report suspected abuse, neglect, or domestic violence
  • For health oversight activities, audits, and investigations
  • To avert a serious threat to health or safety
  • For specialized government functions, including military, national security, and protective services
  • For workers' compensation purposes as authorized by state law
  • To coroners, medical examiners, and funeral directors as permitted by law
  • For organ and tissue donation
  • For research, with appropriate approvals and safeguards

Uses and disclosures that require your written authorization

Other uses and disclosures of your PHI not described in this notice will be made only with your written authorization. This includes most uses or disclosures of psychotherapy notes, uses and disclosures of PHI for marketing purposes, and any sale of PHI. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.

Your rights regarding your health information

You have the following rights with respect to your PHI:

  • Right to access. You have the right to inspect and obtain a copy of your PHI in our designated record set, including your imaging studies and reports, with limited exceptions. Requests should be directed to the imaging center where your study was performed.

  • Right to request restrictions. You have the right to request restrictions on certain uses and disclosures of your PHI. We are not required to agree to your request, except in limited circumstances (for example, when you have paid for an item or service out of pocket in full and request that the information not be disclosed to your health plan).
  • Right to request confidential communications. You have the right to request that we communicate with you about your health information in a specific way or at a specific location, when reasonable.
  • Right to a paper copy of this notice. You have the right to receive a paper copy of this notice at any time, even if you have agreed to receive it electronically.
